Quite a bit of fallout regarding Cam Smiths actions around the tackle. It's quite a big deal in the media down here.
So it should be. Totally on McKinnon's side on that one. Smith is a horrible individual and although there was no intent in the tackle, there is a way to handle it afterwards. Complaining about the penalty and blaming McKinnon for so long when the player was clearly badly injured was just out of order and completely disrespectful. To then not get in contact afterwards was also really poor, especially given Smith's position both at Melbourne and with the national team.

I'm totally disgusted by Smith. I thought he was the biggest grub in the game before this. He is a great great talent, maybe one of the best but his biggest asset is being a sly, sneaky, cheating ba$tard who has the refs hood winked.

At that time, he never knew that McKinnon was disabled. Nobody did. He also did get in contact - Melbourne asked to visit McKinnon several times in hospital, each of those denied.

Everyone knew immediately that it was serious. The nine medical staff around him working furiously was a dead giveaway. Smith showed a complete lack of empathy with a fellow human being in trouble, and his reactions to this day show the same thing. Even if he believes he has done nothing wrong at all to this point, just the fact that McKinnon and his family are obviously distressed means he should, as a human being with feelings, apologise to the family as long and as loud as he can. There has always been something cold and distant about Smith, but his the lack of empathy he has shown and continues to show in this is chilling. At the time he showed he didn't care. In the weeks afterwards he showed he didn't care. Right now he is still showing no concern for anything other than his reputation.

As it is every single time a player is stretched off the pitch, yet how often do you see captains argue that a swinging arm wasn't intentional, that the attacker slipped and so on. He was just defending his player, as a captain is expected to. He led the Storm players in requesting visits to McKinnon several times (all denied), he lauched the Rise For Alex round with Gallen, he even wore McKinnon's player number with his shirt being auctioned off afterwards. How does that give any indication he doesn't care? McKinnon has never raised any issue with Smith until his 60 minutes interview/book promotion, a year later. Smith wasn't even allowed a right of reply. Given how much Smith's actions have distressed him, why hasn't he brought it up at any earlier time?

Bit harsh to link it purely to the book. He had a lot on his mind adjusting to the change and getting over the shock of it all. Blanking out the media for a large part of it probably saved his true opinions about Smith from coming out.

Trying to visit Alex while he was in an induced coma or only allowed visits from family (as was the case till he was stable enough to be moved to Sydney) makes no sense at all. Even his coach and team mates couldn't visit him unless taken in one at a time by family, and he certainly wasn't mentally ready to deal with anyone else. When he was able to be contacted, the player involved in the tackle made the effort to get in touch. Smith didn't. Right now, Alex shares office space with Anthony Quinn, a former team mate of Smith's, so it would be easy for Smith to contact him if he wanted to. He hasn't. All the other stuff you mention is damage control initiated by the NRL.
Smith said what he did in the heat of the moment when as far as he's aware he's trying to win a game. If he hadn't made the comment would people be calling for him to visit McKinnon?
I'm also not sure about McKinnon talking about suing the NRL for the tackle. I can understand him wanting financial security, but IIRC he's been pretty much given that already through various means. The sport has done a lot to try to help him - rightly - and it would seem a bit of a slap in the face to sue the game on top of that.
